Documents Required

1-Passport sized picture.
2-Picture of front page of your Passport.
3-Additionally, you must provide CNIC number and name of a relative who is a Pakistani national Only (Child,Siblings,paternal and maternal grandparents,and paternal and maternal grandchilds)
1-First ever NICOP card
1-Valid copy of Pakistani or Foreign passport.
2-Birth certificate with father’s or mother’s name in case of foreign birth OR
3-S-1 form in case when applicant’s Pakistani passport is expired or invalid.
Copies of parent’s CNIC.
4-Passport is exempted if NICOP is applied for within first six months of birth.

1-Expired NICOP is required.

Additional document for Nicop card
1-You must provide CNIC number and name of a
relative who is a Pakistani national.
2-NICOP/CNIC name and number of parents.

1-While applying for modification in NICOP, the old NICOP has to be shredded
and the photograph of shredded NICOP needs to be sent in regardless of the
nature of circumstances.
2-Additionally, you must provide CNIC number and name of a relative who is a
Pakistani national.
3-Passport sized picture.
4-NICOP/CNIC name and number of parents.
5-Picture of front page of your Passport.
For Change in Martial status:
1-Married Male Nikah Nama OR Affidavit is required.
2-Married Female Nikah Nama OR Affidavit duly countersigned by the husband.
Husband’s CNIC/MNIC/NICOP OR confirmation through email by the
husband along with the copy of CNIC/MNIC/NICOP.
3-Divorced Male/Female
Divorce Papers or the issued certificate.
4-Deceased Husband
Death Certificate of husband is required.
5-For change of family head
CNIC / NICOP of head of the family or guardian
